What Causes a Sandpaper Rash?

A rough, fine-bump texture may occur with atopic dermatitis, contact dermatitis, heat rash, or intertrigo in warm or damp folds. Psoriasis can produce thicker, dry scale, while ringworm may form an expanding scaly edge. Scabies often causes intense itching, especially at night, with bumps in characteristic areas. A drug rash can begin after a new medicine and may spread beyond the original area.

The feel of the skin is only one clue. Location, itch, pain, fever, new products, medicines, sweating, and contact with other affected people help distinguish these conditions. A rash should not be labelled from texture alone.

Use lukewarm water, a fragrance-free cleanser, and a plain moisturiser. Keep sweaty skin cool and dry, and stop any newly introduced cosmetic, detergent, or topical product until the cause is clearer. Do not scratch, scrub, or apply steroid or antifungal medicine casually, because the wrong product can alter the appearance or worsen some infections. Record when the rash began and any new exposure.

Seek urgent care for a rash with breathing difficulty, facial swelling, blistering, skin peeling, severe pain, purple spots, confusion, or fever with rapid spread. Arrange a clinical review when the rash persists, recurs, affects the eyes or mouth, becomes infected-looking, or follows a new medicine. Infants, older adults, and people with reduced immunity should seek advice sooner when symptoms are significant.
