Causes of Plica Syndrome

What causes plica syndrome?
A plica is a normal fold of synovial tissue inside the knee. It can thicken or become sensitive after repeated friction, a sudden rise in running or cycling load, frequent squatting or stairs, a blow to the front of the knee, or a period of altered movement after another knee problem.
What an irritated plica can feel like
Plica syndrome often presents with pain toward the inner front of the knee, tenderness beside the kneecap, and a catching or snapping sensation during bending and straightening. Symptoms may build with repeated stairs, squats, running, or sitting with the knee bent.
Knee clicking is also common without plica irritation, and pain at the front of the knee can arise from the kneecap, tendon, cartilage, or other joint structures. True locking, where the knee cannot fully bend or straighten, suggests a mechanical obstruction that needs timely assessment.
How to reduce irritation
Temporarily reduce the movements that repeatedly provoke pain, then maintain comfortable walking and gentle knee motion. Avoid repeatedly testing a painful click. A wrapped cold pack may help after activity, while footwear and training changes can reduce an abrupt load on the knee.
If symptoms persist or limit activity, physiotherapy may examine hip and knee strength, kneecap movement, flexibility, and training load. Rehabilitation can then build capacity gradually. Knee arthroscopy is considered only after the diagnosis and response to non-surgical care have been reviewed by an appropriate specialist.
When the knee needs medical review
Arrange prompt care if the knee truly locks, repeatedly gives way, cannot bear weight, becomes markedly swollen after injury, or fails to regain movement. These features can point to a problem beyond an irritated plica.
Seek urgent assessment for a hot, red, rapidly swollen knee with fever or severe illness, an obvious deformity after trauma, or a cold, pale, numb foot. Do not try to force a locked or deformed knee straight.

No. Knees can click from several tissues and may make sounds without pain during normal movement. Plica syndrome is considered when the location, tenderness, activity pattern, and examination fit.
Can running cause plica syndrome?
A rapid increase in running or repeated knee bending can irritate a susceptible plica. Running is not automatically harmful; adjusting load and addressing movement deficits may allow a gradual return.
