Understanding night-time episodes

A panic attack can wake someone with a sudden surge of fear, racing heart, sweating, shaking, chest tightness, breathlessness, dizziness, or numbness and tingling. It may occur without a dream or obvious danger. Stress held throughout the day, irregular sleep, caffeine, nicotine, alcohol, some medicines, and concern about bodily sensations can lower the threshold for an episode. Reflux, breathing problems during sleep, low blood sugar, and heart rhythm symptoms can feel similar, so repeated or unfamiliar attacks should not be self-diagnosed. Panic disorder is considered when attacks recur with ongoing concern or avoidance.

What to do during and after an attack

Sit upright or place your feet on the floor, loosen tight clothing, and breathe gently with a longer exhale rather than taking rapid deep breaths. Name the sensations and remind yourself that the surge will pass, while checking for symptoms that are unusual for you. Keep a brief diary of sleep, stress, substances, medicines, and symptoms. A GP can review physical causes and discuss talking therapy or other options; a mental health assessment can help clarify recurring episodes.

When to seek urgent care

Call emergency services for severe or persistent chest pain, major breathing difficulty, fainting, blue lips, new weakness, confusion, or symptoms that feel different from previous panic attacks. Seek prompt medical advice for repeated night waking, loud snoring with gasping, substance withdrawal, thoughts of self-harm, or increasing avoidance of sleep. Do not assume every episode is panic when a physical warning sign is present.
