What can cause one swollen ankle?

A recent twist, fall, impact, long journey, wound, insect bite, new exercise, or sudden painful joint can provide useful context. The cause cannot be judged from swelling alone, especially when there was no clear injury.

Clues that help narrow the cause

An ankle sprain commonly presents with swelling, bruising, and pain after the foot rolls. An ankle fracture may present after trauma with marked tenderness or inability to bear weight. Gout can present as a rapid onset of intense joint pain, warmth, redness, and swelling. Osteoarthritis more often produces activity-related pain and stiffness with intermittent swelling.

Skin warmth, spreading redness, a break in the skin, or fever raises concern for infection. Swelling that extends into the calf, particularly with calf pain or tenderness, needs prompt evaluation for a clot. These patterns guide urgency but do not replace an examination.

After a minor injury, protect the ankle from painful loading, elevate it when resting, and use a wrapped cold pack briefly for comfort. Keep the toes and knee moving if comfortable. Do not tightly bandage an ankle that is numb, discoloured, unusually cold, or increasingly painful.

Seek a clinical assessment if you cannot walk, tenderness is sharply focused over bone, swelling appeared without injury, or it is not settling. A clinician may examine the ankle, calf, skin, circulation, and joint movement, then decide whether imaging or other tests are needed.

When to get urgent help

Get urgent medical care for one-sided ankle or leg swelling with chest pain, sudden shortness of breath, coughing blood, fainting, or a rapid heartbeat. Also seek urgent assessment when the calf becomes swollen and painful without a clear injury.

Immediate evaluation is warranted for a cold, pale, blue, or numb foot; a visibly deformed ankle; rapidly spreading redness; severe pain out of proportion to the injury; or fever with a hot swollen joint.
