Causes of Morphea

Morphea can begin as a patch that looks pink, purple, or lighter than nearby skin and later becomes firmer, smoother, or darker. The process affects local skin and connective tissue rather than behaving like the widespread organ involvement associated with systemic scleroderma. It is not considered contagious.

The cause is often not identifiable. A recent injury, procedure, infection, or other local event may precede a patch, but timing alone does not prove that event caused it. The depth, location, symptoms, and signs of activity guide assessment.

Arrange dermatology review for a new firm or tightening patch, particularly if it is expanding, itchy, painful, or restricting movement across a joint. Take photographs in consistent light and note changes in colour, texture, and flexibility. Treatment may be topical for limited superficial disease; Topical Calcineurin Inhibitors or Narrowband UVB Phototherapy may be considered in selected situations.

Seek prompt assessment if a patch is quickly enlarging, crosses a joint, affects the face or scalp, causes pain or weakness, or leaves a deep indentation. Children and people with lesions near the eyes, mouth, or a major joint should be reviewed early. Urgent care is warranted for sudden severe swelling or functional loss.

Ask whether the skin appears active, how depth and joint movement will be monitored, and whether a biopsy or imaging is useful. Discuss the expected course, treatment effects, skin care, and which changes should lead to an earlier appointment.
