What causes mid-back spasms?
A spasm is an involuntary muscle contraction. It may start after lifting, twisting, prolonged sitting, a new workout, coughing, or sleeping in an awkward position. Sometimes no single trigger is obvious.
What the pattern may suggest
A strained muscle commonly produces local tenderness and pain that increases when you twist, reach, or take a deep breath. Irritation of a small spinal joint may feel one-sided and movement-dependent. An irritated disc can cause pain that wraps around the rib area, although muscle guarding can occur without nerve involvement.
Stress, fatigue, dehydration, and unfamiliar activity may make muscles easier to tighten, but persistent episodes deserve assessment for movement, joint, or nerve contributors. The location of pain alone cannot identify the cause.

Pause the activity that triggered the spasm, then keep gently mobile rather than staying in bed. Short walks, comfortable trunk movements, heat, and supported sitting may ease guarding. Avoid forceful stretching, heavy lifting, or repeated twisting while the area is sharply painful.
Arrange an assessment if spasms keep returning, restrict normal activity, or do not begin to settle. A clinician can check spinal movement, ribs, muscle tenderness, strength, sensation, and whether treatment should focus on load management, mobility, or another cause.
When to seek care
Seek urgent medical care for mid-back pain with new leg weakness, numbness around the groin, loss of bladder or bowel control, severe chest pain, difficulty breathing, fainting, fever with marked illness, or pain after a significant fall or collision.
Prompt assessment is also sensible when pain is constant at rest, repeatedly wakes you, travels around the chest with numbness, or occurs alongside unexplained weight loss or a history of cancer, osteoporosis, or immune suppression.
