What causes an imperforate hymen?

It is a congenital anatomical variation. It is not caused by exercise, sexual activity, tampon use, hygiene, anything done during pregnancy, or anything the child has done.

Before puberty, an imperforate hymen may cause no obvious problem. Once menstrual bleeding begins, blood cannot leave through the vaginal opening and can collect behind the hymenal tissue. This may produce recurring lower abdominal or pelvic pain, pressure, backache, difficulty passing urine or stool, or an apparent absence of periods despite other pubertal development.

An imperforate hymen is different from delayed puberty: hormonal development and menstrual production may be occurring normally while the outflow path is blocked. Clinicians also distinguish it from other structural causes of menstrual obstruction.

Arrange a medical visit if periods have not appeared as expected alongside cyclical pain, or if a clinician has noticed tissue covering the vaginal entrance. Evaluation commonly starts with a careful external examination. A pelvic ultrasound may show retained fluid and help assess the uterus, vagina and nearby structures.

Treatment is a planned minor operation that creates an opening and allows trapped menstrual material to drain. The treating team explains anaesthesia, wound care, expected discharge and follow-up. Do not try to puncture or stretch the tissue at home, because this can cause injury, infection and bleeding.

Seek urgent care for severe or rapidly worsening abdominal pain, inability to pass urine, repeated vomiting, fainting, fever with pelvic pain, or feeling acutely unwell. These symptoms require timely examination whether or not an imperforate hymen has already been suspected.

Recurring monthly pain without visible bleeding is not an emergency when mild, but it should be assessed soon rather than managed repeatedly with pain medicine alone.
