Causes of hip bursitis on both sides

Short answer
Pain labelled as bursitis on both outer hips often reflects repeated compression, tendon overload, altered movement, or wider inflammatory problems.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical area
- The painful point is commonly over the bony outer side of the hip.
- Common trigger
- Direct pressure from side-lying can aggravate symptoms.
- Rehabilitation focus
- Load management and progressive hip strengthening are often central.
What can cause bursitis-like pain in both hips?
The bursae and the gluteal tendons sit close together beside the greater trochanter, so symptoms commonly grouped under hip bursitis may involve either or both tissues. Sleeping on either side, sudden changes in walking or exercise, and reduced hip strength can irritate this area.
Why symptoms may affect both sides
A shared load pattern can expose both hips: prolonged standing with weight shifted outward, crossing the legs, climbing more hills or stairs, or rapidly increasing running distance. Pain on one side may also change the way a person walks and overload the opposite side.
Tenderness over the bony outer hip and pain when lying on that side fit greater trochanteric pain. Pain arising from the lower back or hip joint can feel similar, while widespread morning stiffness or several painful joints may suggest an inflammatory condition that needs separate assessment.
Ways to reduce irritation
Temporarily reduce hills, long walks, repeated stairs, or exercises that provoke outer-hip pain. When sleeping, lie on your back or place a supportive pillow between the knees; avoid letting the upper knee drop across the body. Stand with weight shared between both feet.
A physiotherapist can assess hip strength, walking mechanics, spinal contribution, and training load, then build gradual strengthening. Injection or shockwave treatment may be considered for selected cases after the diagnosis is reviewed, but these options do not replace correcting persistent load triggers.
Symptoms needing medical attention
Arrange prompt evaluation if pain prevents weight-bearing, follows a significant fall, or is accompanied by fever, a hot swollen joint, rapidly spreading redness, or feeling seriously unwell. These features are not typical of simple tendon overload.
New leg weakness, numbness around the groin, or loss of bladder or bowel control needs emergency assessment. Seek a routine review when pain repeatedly disrupts sleep, worsens despite modifying activity, or makes everyday walking progressively harder.
Questions to clarify the diagnosis
Ask whether the main source seems to be a bursa, gluteal tendon, hip joint, or lower back. Discuss which activities are driving compression, whether imaging would alter treatment, and how to judge a safe increase in walking or strengthening.

Questions people ask
Does pain on both sides confirm two inflamed bursae?
No. Outer-hip pain may come from nearby gluteal tendons, the hip joint, the lower back, or a combination, so examination matters.
Should I stretch through outer-hip pain?
Forceful stretches that pull the knee across the body can compress irritated outer-hip tissues. Gentle movement and a tailored strengthening plan are usually more appropriate.
