Causes of Heat Hives

Short answer
Heat hives occur when a rise in body temperature and sweating triggers small itchy welts, often after exercise, warmth, or emotional stress.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common triggers
- Exercise, warmth, stress, fever, and hot or spicy food can raise body temperature.
- Typical sensation
- Small itchy welts may feel prickly or burning.
How heat hives behave
The spots may be tiny and surrounded by redness, with a prickling or burning itch. They often start on the trunk or neck and can spread while the trigger continues. Warm showers, fever, crowded rooms, spicy food, and strong emotions may produce the same temperature or sweating response. Individual welts usually fade as the skin cools, although episodes can recur.
Heat hives differ from heat rash, which relates to blocked sweat ducts. A clinician can compare the timing, appearance, and duration with other forms of urticaria.
Reducing episodes
Move to a cooler place, loosen clothing, rinse with lukewarm or cool water, and avoid scratching. Choose breathable layers and build exercise intensity gradually. Keep a brief record of heat, exertion, food, stress, and medicines before an episode. A clinician may discuss an oral antihistamine and how to use it safely, especially if you have other conditions or take regular medicines.
When to seek urgent care
Call emergency services for hives with swelling of the lips, tongue, or throat, wheezing, trouble breathing, faintness, or repeated vomiting. Arrange a medical review when episodes are frequent, last beyond the expected trigger, disturb sleep, or occur without heat or sweating. Fever with a widespread or painful rash also needs assessment.
Questions for your doctor
Ask whether the pattern fits cholinergic urticaria, which triggers to test or avoid, whether an antihistamine is suitable for you, and what emergency symptoms should change your plan.

Questions people ask
Are heat hives the same as heat rash?
No. Heat hives are a hive response linked to temperature rise or sweating; heat rash involves irritated, blocked sweat ducts.
Can antihistamines help?
An oral antihistamine may reduce symptoms for some people, but the choice and dose should be discussed with a clinician.
