Causes of groin pain

Short answer
Groin pain can arise from muscles, tendons, the hip joint, a hernia, or nearby urinary and reproductive organs.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Useful clue
- Pain with a specific movement can help identify the involved structure.
- Assessment
- Hip, abdominal, and groin examinations may all be relevant.
- Avoid
- Do not push through sharp pain or forceful stretching.
What causes groin pain?
A sudden strain is more likely after sprinting, kicking, twisting, or changing direction. Joint-related pain may feel deeper and may come with stiffness, clicking, or reduced hip movement. The pattern helps narrow the cause, but location alone cannot confirm it.
What the pain pattern may mean
Pain near the inner thigh that increases when squeezing the legs together can fit an adductor injury. A deep ache with bending or rotating the hip can occur with hip impingement, a labral problem, or osteoarthritis. A bulge that becomes clearer with coughing or straining can suggest a hernia.
Burning urination, abdominal discomfort, testicular symptoms, pelvic symptoms, fever, or nausea point beyond a simple muscle injury and deserve medical assessment. More than one structure can be irritated at the same time.
What to do next
Pause the movement that triggered the pain and choose gentle walking only if it does not worsen symptoms. A wrapped cold pack may ease discomfort after a recent strain; avoid placing ice directly on skin. Do not force a deep groin stretch during sharp pain.
Arrange an assessment if pain persists, keeps returning, limits normal walking, or follows a significant injury. Note the exact location, onset, provoking movements, any lump, and associated urinary, abdominal, or hip symptoms. Those details guide the examination and whether imaging is useful.
When to seek urgent care
Seek urgent care for severe pain after trauma, inability to bear weight, a visibly deformed hip, or a painful groin lump that cannot be gently reduced. Sudden severe testicular pain, marked swelling, vomiting, faintness, fever, or rapidly worsening pain also needs immediate assessment.
New numbness around the groin, loss of bladder or bowel control, or leg weakness requires emergency evaluation because these can signal pressure on important nerves.
Questions for your clinician
Ask which structure appears to be painful, whether the findings suggest a hip, tendon, hernia, or non-musculoskeletal cause, and what activities are safe. You can also ask whether physiotherapy is appropriate and which change in symptoms should prompt reassessment.

Questions people ask
Can groin pain come from the hip?
Yes. Hip impingement, labral injury, and osteoarthritis can present with deep groin discomfort, often alongside stiffness or pain during hip rotation.
Is every episode of groin pain a muscle strain?
No. Tendons, the hip joint, hernias, and urinary or reproductive organs can produce pain in the same region, so persistent or unexplained symptoms need assessment.
