Elbow and forearm pain often comes from irritated tendons or strained muscles, but joints, nerves, and injuries can also be responsible. Repeated gripping, lifting, twisting, racquet sports, tool use, or a sudden increase in activity can overload the tissues joining the forearm muscles to the elbow. Pain after a fall or direct blow needs assessment for structural injury.
Clues from location and movement
Tennis elbow commonly presents with pain at the outer elbow that is aggravated by gripping, lifting with the palm down, or extending the wrist. Golfer's elbow usually presents with pain at the inner elbow during gripping, wrist bending, or forearm rotation. Either condition may spread discomfort into the forearm, and neither requires playing the sport in its name.
A muscle strain is more likely after a forceful effort and may hurt when that muscle contracts or stretches. Tingling, numb fingers, burning pain, or reduced hand strength can suggest nerve irritation at the elbow, forearm, wrist, or neck. Deep joint pain, locking, marked swelling, or restricted motion raises other possibilities, including arthritis or an injury inside the joint.
How to respond
Temporarily reduce the movement or load that provokes pain rather than stopping all arm use. Adjust grip size and technique, take breaks from repetitive work, and keep the elbow and wrist moving gently within a comfortable range. A wrapped cold pack can be used briefly after an aggravating activity. Check with a pharmacist or clinician before using pain medicine if you have other health conditions or take regular medicines.
Arrange a clinical or physiotherapy assessment when pain persists, keeps returning, affects sleep or work, or is accompanied by weakness or altered sensation. The examination can compare the neck, elbow, wrist, tendons, muscles, and nerves. Imaging is not required for every overuse problem, but may help after trauma or when the findings suggest bone or joint damage.
Signs that need prompt care
Seek urgent help after an injury if the arm is deformed, a bone is visible, swelling is rapidly increasing, or you cannot move the elbow. Prompt assessment is also needed when the hand becomes cold, pale, blue, very weak, or numb. A hot swollen elbow with fever or feeling unwell may indicate infection. Call emergency services if arm discomfort accompanies chest pressure, breathlessness, sweating, or sudden weakness on one side.
What to ask during assessment
Consider asking which structure appears irritated, whether symptoms could originate from a nerve or the neck, and which work or sport movements should be modified. Ask for a graded plan to rebuild grip and forearm capacity, guidance on supports if appropriate, and clear signs that would justify imaging or an earlier follow-up.
