What should you know about causes of early puberty?

In central early puberty, the brain and pituitary gland switch on the same pathway used in typical puberty, but sooner. Often no underlying disease is found. In some children, a brain or nervous-system condition, previous treatment affecting the brain, or a developmental difference can trigger the pathway.

Peripheral early puberty bypasses the brain's signal. The ovaries, adrenal glands, or another source produces sex hormones independently. Causes can include ovarian or adrenal disorders and hormone-containing creams, gels, tablets, or supplements used by the child or another household member. Breast or pubic-hair development alone does not always mean the full puberty pathway is active.

Make an appointment when pubertal changes appear unusually early, advance quickly, or cause distress. Record when breast development, body hair, acne, body odour, growth acceleration, bleeding, or voice change began. Bring a list of all medicines, supplements, and hormone products used in the home, including skin products that could transfer by contact.

The clinician may compare height over time, assess the sequence of physical changes, and check bone maturation. Imaging is selected from the history, examination, and test results rather than being required for every child.

Seek prompt medical assessment if early pubertal changes occur with severe or persistent headache, vomiting, seizures, weakness, problems with balance, or new vision changes. These neurological symptoms can indicate a cause that needs timely investigation.

Unexpected vaginal bleeding, rapidly progressing physical changes, significant abdominal pain or swelling, or exposure to a hormone medicine also warrants an early appointment. Use emergency services for severe pain, collapse, breathing difficulty, or a seizure that is prolonged or repeats without recovery.

Ask whether the pattern represents central puberty, a peripheral hormone source, or an isolated change. Clarify which tests can distinguish them, how growth and bone maturation will be monitored, whether treatment is appropriate, and how to support the child's privacy and emotional wellbeing at school and home.
