The combination deserves attention when pain is new, worsening, one-sided, or accompanied by fever, unusual discharge, dizziness, or a possible pregnancy. Timing and associated symptoms help narrow the possibilities.
Clues in the pattern
Brief discomfort and light spotting around the middle of a cycle can accompany ovulation. Hormonal contraception, missed doses, or a recently fitted contraceptive device may also alter bleeding and cramps. Chlamydia and other pelvic infections can present with pelvic pain, irregular bleeding, discharge, or pain during sex.
Uterine fibroids and endometrial polyps can present with bleeding between periods and cramping.
