What causes lower back muscle spasms?

Short answer
Lower back muscle spasms are involuntary contractions commonly triggered by muscle strain, fatigue, awkward movement or protective tightening around an irritated spinal structure.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- What it is
- An involuntary contraction of muscles in the lower back
- Common triggers
- Strain, fatigue, sudden movement and protective muscle guarding
- Key point
- The spasm itself does not reveal the exact underlying cause
What causes lower back muscle spasms?
A spasm is a response rather than a diagnosis. It can develop after lifting, sudden twisting, prolonged positioning or unfamiliar activity. In some cases, pain from a disc or facet joint prompts nearby muscles to tighten, but the sensation of spasm alone cannot identify which tissue started the problem.
What a back spasm means
During a spasm, bands of muscle contract suddenly and may make the back feel locked. Pain can intensify with movement, and standing upright may be difficult for a while. This protective reaction does not by itself show how serious the underlying issue is.
Muscle strain and non-specific low back pain can present with spasm. Facet joint irritation or a herniated disc can also be accompanied by muscle guarding, especially when other features such as pain travelling into a leg are present. An examination uses the full symptom pattern, strength, sensation and movement rather than the spasm alone.
Steps that may help
Pause the movement that triggered the episode and choose comfortable positions, but avoid staying completely still for long periods. Gentle walking and gradual return to usual activity are often easier than forceful stretching during an active spasm. Heat or a wrapped cold pack may be used according to comfort, while protecting the skin.
If spasms recur, a physiotherapist can assess lifting technique, hip and trunk movement, strength and workload. Hands-on approaches such as myofascial release or dry needling may be considered for selected people as part of a broader plan; they do not replace assessment of persistent pain or neurological symptoms. Check with a pharmacist or clinician before using pain medicine if you have other health conditions, take regular medicines or are pregnant.
When to seek medical care
Seek urgent care for back pain with new difficulty controlling the bladder or bowel, numbness around the genitals or inner thighs, or significant weakness in a leg. Urgent assessment is also appropriate after a major injury, or when severe back pain occurs with fever or feeling acutely unwell.
Arrange a routine assessment when episodes keep returning, pain is not settling, sleep or daily tasks remain disrupted, or pain travels below the knee with tingling or numbness. These details help the clinician decide whether further investigation or a tailored rehabilitation plan is appropriate.
Questions for your clinician
Describe the movement or activity before the spasm, where the pain spreads and whether you notice weakness, numbness or bladder changes. Ask which movements are suitable now, how to rebuild normal activity, what may be driving repeat episodes and which specific changes would require urgent review.

Questions people ask
Does a lower back spasm mean I have a herniated disc?
No. A herniated disc can be accompanied by muscle guarding, but strain and non-specific low back pain can also present with spasm. Leg pain, altered sensation, weakness and examination findings provide more useful clues.
Should I stretch a back spasm?
Gentle, comfortable movement may help, but forcing a painful stretch can increase guarding. Stop if pain sharply worsens or travels down the leg, and seek an assessment if movement remains very limited.
Why do my back spasms keep returning?
Repeat episodes may reflect an activity or load that exceeds current capacity, incomplete recovery, movement habits or ongoing irritation. A clinical assessment can look for a pattern and guide graded strengthening and activity changes.
