Cast for a femur fracture: what to know

Because the thigh bone carries major loads and strong muscles can pull fragments out of alignment, many femur fractures require surgical fixation. A hip spica cast may be chosen for some children or particular stable injuries after alignment has been assessed.

A hip spica surrounds part of the trunk and the injured leg to limit motion at the hip and fracture. Its shape makes ordinary sitting, toileting, clothing and transport difficult, so caregivers need practical instruction before discharge. Openings must remain clean and padding must stay dry.

The cast does not remove the need for monitoring. Children grow, swelling changes and fragments can shift, making scheduled clinical and imaging checks central to safe care.

Follow the team's demonstrated lifting and turning technique, supporting the cast rather than using its crossbar as a handle. Change position regularly as instructed, check visible skin around every edge, and keep crumbs, toys, powders and lotions out. Protect the cast during toileting without blocking airflow.

Never allow standing or weight bearing unless specifically permitted. Use only a transport arrangement approved for the cast's width and hip position, and keep every follow-up appointment even when pain has eased.

Seek emergency care if the toes become cold, blue, very pale, numb or difficult to move, or if severe pain continues despite the advised measures. Sudden breathing difficulty or chest pain after a major leg injury also requires emergency help.

Call the orthopaedic service promptly for a cast that cracks, softens, becomes soaked or develops a foul smell. Increasing swelling, burning, pressure pain, fever, discharge, skin sores, or urine and stool trapped under a hip spica all warrant assessment.

Ask why casting suits this fracture, how alignment will be monitored, which positions are allowed, and how to manage hygiene and transport. Confirm when weight bearing may begin, whether another support will follow, and how hip and knee movement will be restored.
