Cast for an avulsion fracture: what to know

The choice depends on the fragment's location, displacement, joint stability and your ability to avoid loading the injured area. A splint or walking boot may suit other injuries, while a markedly displaced or unstable fragment may require a different treatment.

An avulsion fracture occurs where a tendon or ligament pulls a small piece of bone away. Immobilisation limits movement at that attachment and can reduce repeated traction on the fragment. The cast may span a nearby joint, so stiffness and temporary muscle weakness can develop during healing.

Follow-up imaging and examination help the clinician check alignment and decide when support can be reduced. Do not use pain relief alone as a sign that the bone is ready for unrestricted activity.

Keep the cast dry, protect its edges and never place objects inside to scratch. Elevate the injured limb during early swelling, and move uncovered fingers or toes as instructed. Follow the exact weight-bearing restriction: a lower-limb cast should not be walked on unless the treating team says it is designed for that.

Attend scheduled checks because swelling can change the fit. After immobilisation, a gradual return of joint movement, strength and balance may be recommended rather than an immediate return to sport.

Seek urgent care if pain becomes severe or keeps increasing despite elevation and prescribed medicine, or if exposed fingers or toes become numb, weak, unusually cold, pale or blue. These changes can signal pressure on nerves or circulation.

Arrange prompt assessment for a cracked, softened, wet, loose or foul-smelling cast, burning beneath it, spreading redness, fever, or fluid coming through the material. Do not cut or reshape the cast yourself.

Ask whether the fragment involves a joint, when you may bear weight or use the limb, how follow-up will confirm healing, and which movements remain safe. Also clarify whether you will change to a removable support and what rehabilitation is expected afterward.
