Carpal Tunnel and Hand Swelling

People sometimes describe the hand as puffy or tight even when little swelling is visible. A clinician can separate altered sensation from true fluid swelling and look for wrist, joint, tendon, circulation or inflammatory causes.

Carpal tunnel syndrome occurs when the median nerve is compressed at the wrist. Its characteristic symptoms affect the thumb, index, middle and part of the ring finger, may disturb sleep, and can make gripping or fine hand tasks difficult.

Visible puffiness across the hand or fingers is not enough to identify the cause. Rheumatoid arthritis can present with swollen, tender joints and stiffness, while a distal radius fracture can present with pain and swelling after a wrist injury. Other causes include tendon irritation, infection and reduced fluid drainage.

Remove rings early if swelling is developing. Rest from activities that clearly worsen symptoms, keep the wrist in a comfortable neutral position, and record whether numbness follows a particular finger pattern or wakes you at night.

Arrange an assessment if symptoms persist, return repeatedly, or interfere with sleep or hand function. Examination may compare sensation, strength, joint movement, tenderness and swelling; testing is selected according to the history rather than from swelling alone.

Seek urgent assessment after a significant injury, or if swelling is rapid and severe, a ring becomes trapped, the hand changes colour or feels cold, or you cannot move the fingers normally. Sudden loss of strength or sensation also warrants prompt help.

Redness, warmth, spreading tenderness, drainage, fever, or swelling after a bite or wound may signal infection and should be assessed quickly. Breathing difficulty or swelling of the face or tongue requires emergency care.

Ask whether the swelling is in a joint, tendon area or soft tissue; whether the sensory pattern fits median nerve compression; and which examination findings guide the diagnosis. Discuss safe symptom relief, work or sleep adjustments, and what change should prompt earlier follow-up.
