Can spinal stenosis cause cauda equina syndrome?

The risk arises when narrowing in the lower spinal canal becomes tight enough to press on the bundle of nerves below the spinal cord. Not everyone with spinal stenosis develops this complication.

The cauda equina is a group of nerve roots that controls feeling and movement in the legs as well as bladder, bowel and sexual function. Marked canal narrowing can crowd these roots and interrupt their signals.

Cauda equina syndrome may present with new difficulty starting urination, inability to empty the bladder, loss of bladder or bowel control, numbness around the genitals or buttocks, or new weakness in one or both legs. Back pain or pain travelling down a leg may occur, but pain by itself cannot confirm the syndrome.

If any bladder, bowel, saddle-area sensation or new leg-weakness warning sign is present, go to an emergency department now. Do not wait for a routine clinic visit, physiotherapy session or a planned scan. Avoid driving yourself if leg weakness, altered sensation or bladder problems could make driving unsafe.

At the hospital, describe when each symptom began and whether it is worsening. The clinical team may check leg strength, sensation and reflexes, assess bladder function, and arrange urgent spinal imaging. Treatment depends on what is compressing the nerves and may include urgent surgery to relieve pressure.

Seek immediate emergency care for new numbness between the legs or around the genitals, difficulty sensing or controlling urination or bowel movements, inability to pass urine, or rapidly developing leg weakness. These symptoms are urgent because ongoing nerve compression can damage functions served by the affected roots.

If you have spinal stenosis with pain or tingling but none of these red flags, arrange a clinical review if symptoms are new, worsening, affecting walking, or limiting daily activity. A clinician can assess whether the pattern fits ordinary nerve irritation or needs faster investigation.

Ask which changes in bladder control, bowel control, saddle-area feeling or leg strength should send you directly to emergency care. You can also ask where the spinal canal is narrow, which nerves may be affected, whether imaging is needed, and which activities or medicines are appropriate while you await assessment.
