Can smouldering myeloma cause multiple myeloma?

Smouldering myeloma is an early, symptom-free stage in which abnormal plasma cells or their proteins are present without the organ damage that defines active myeloma. It requires planned follow-up rather than assumptions about what will happen next.

Progression means the plasma-cell disorder has become active and is causing myeloma-defining changes. Clinicians assess blood and urine findings, bone marrow results, kidney function, calcium, blood counts, and imaging. A changing laboratory result does not by itself tell the whole story, so results are interpreted together and compared over time.

The chance of progression differs between people. The amount and pattern of abnormal protein, the proportion and features of plasma cells, and imaging findings help the haematology team judge an individual's risk and choose a monitoring schedule.

Keep every haematology appointment even when you feel well, and complete requested blood, urine, or imaging tests. Bring a current medicines list and report new symptoms rather than waiting for the next routine visit. Do not start supplements or anti-inflammatory pain medicines without checking whether they are suitable for your kidneys and current treatment plan.

Ask which changes your team is watching, how you will receive results, and whom to contact between appointments. Monitoring aims to identify active disease promptly while avoiding treatment before it is needed.

Contact your care team promptly for persistent or severe bone pain, an unexplained fracture, marked tiredness, repeated infections, reduced urine, increasing thirst, nausea, confusion, or new weakness or numbness. These symptoms have several possible causes, but they need assessment because they can accompany active myeloma or its complications.

Seek emergency care for sudden leg weakness, loss of bladder or bowel control, severe confusion, difficulty breathing, or rapidly worsening illness. Do not wait for a scheduled monitoring appointment when these signs appear.

Consider asking: Which results place me in my current risk group? What tests will be repeated, and what changes would prompt further imaging or a bone marrow test? Which symptoms should lead me to call the clinic the same day? Would a clinical trial or specialist review ever be appropriate for my situation?
