The superior vena cava carries blood from the head, neck, arms and upper chest back to the heart. A tumour or enlarged lymph nodes in the chest can narrow this vein; slower blood flow may also encourage clot formation. Blood then backs up above the narrowed area, causing puffiness of the face or neck, swollen arm veins, visible veins across the upper chest, breathlessness, cough, headache or a feeling of fullness in the head.

Contact the oncology team promptly if new upper-body swelling, breathlessness or prominent chest veins develop. Clinicians may check oxygen levels and arrange chest imaging to locate the narrowing and look for a clot. Treatment is chosen according to symptom severity and the cause; options can include cancer treatment to shrink the mass, medicine for a confirmed clot, or a stent to reopen the vein. Keep the head raised while awaiting assessment if lying flat makes breathing or pressure symptoms worse.

Seek emergency care for severe or rapidly worsening breathlessness, noisy breathing, fainting, confusion, marked drowsiness, blue-grey lips, or sudden swelling of the face and neck. These signs can indicate major restriction of blood return or pressure affecting the airway or brain. Do not drive yourself if breathing is difficult or consciousness is affected.

Ask what is narrowing the vein, whether imaging shows a clot, how quickly treatment should begin, and which option is expected to relieve the obstruction. It can also help to ask which changes in breathing, swelling, headache or alertness should trigger emergency care and whom to contact outside clinic hours.
