The short answer
The two conditions are distinct, and tendinitis does not inevitably progress to frozen shoulder. Stiffness may develop when the shoulder capsule becomes inflamed and tight after the arm has been moved less because it hurts.
Telling tendinitis and frozen shoulder apart
Rotator cuff tendinitis commonly causes pain with lifting, reaching or lying on the affected side. Movement may feel difficult because of pain, but another person can often move the relaxed arm farther than you can move it yourself.
Frozen shoulder causes a progressive restriction in both active movement and movement performed by an examiner. Reaching behind the back and turning the arm outward often become particularly limited. Because pain and stiffness can occur in both conditions, a physical examination is useful before assuming one has turned into the other.
Protect movement without provoking pain
Modify repeated overhead loading and other actions that sharply increase pain, while continuing comfortable daily shoulder movement. Complete rest can allow stiffness to build. A physiotherapist can select gentle range exercises and later add cuff and shoulder-blade strengthening without repeatedly irritating the tendon.
If movement is already tightening, arrange a clinical review. Treatment may include tailored exercises, pain relief or an injection after your medical history and examination are considered. Forceful stretching is not appropriate when it produces significant or lasting pain.
When assessment matters
Book an appointment if shoulder movement is steadily decreasing, night pain disrupts sleep, symptoms interfere with dressing, or home measures are not helping. Seek urgent care after trauma if the shoulder is misshapen or the arm cannot be moved, and for fever with a hot red joint, sudden marked swelling, or a cold, pale or persistently numb hand.
