Can rotator cuff tear cause shoulder osteoarthritis?

Short answer

Yes. A large or long-standing rotator cuff tear can alter shoulder mechanics and contribute to a form of shoulder osteoarthritis.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

A major tear can disturb joint mechanics and increase uneven cartilage loading.
Assessment
Strength, movement and imaging can help identify whether tendon damage and arthritis coexist.
Treatment focus
Relieve pain, preserve useful movement and match treatment to function and joint damage.

The short answer

This does not mean every tear leads to arthritis. The connection is more relevant when the tendon damage is extensive and the muscles can no longer keep the ball of the shoulder centred in its socket.

How the conditions connect

The rotator cuff muscles guide and stabilise the shoulder during movement. When a major tear stops them working effectively, the upper arm bone may shift upward and load the joint unevenly. Over time, cartilage can wear and the joint may become arthritic; clinicians may call this cuff tear arthropathy.

Pain from a tear and pain from arthritis can overlap. Weakness when lifting or rotating the arm may suggest tendon dysfunction, while stiffness, grinding and a deep joint ache can accompany arthritis. An examination and imaging help distinguish these problems and show whether both are present.

What you can do next

Arrange an assessment if shoulder pain, weakness or loss of movement is limiting sleep, dressing, work or exercise. A clinician can check active and passive movement, tendon strength and joint tenderness, then decide whether an X-ray, ultrasound or MRI would change management.

Until assessed, adapt painful overhead tasks rather than repeatedly forcing through weakness. Physiotherapy may focus on comfortable mobility, shoulder-blade control and the muscles that still function. Pain medicines or injections may be considered after checking your health history. Surgery is considered according to tear pattern, arthritis severity, function and treatment goals.

When to seek medical care

Seek prompt assessment after an injury if you cannot lift the arm, the shoulder looks deformed, or pain is severe. Urgent care is also appropriate for a cold or pale hand, new widespread numbness, marked swelling, fever with a hot red shoulder, or rapidly worsening symptoms.

For symptoms that build gradually, book a routine appointment when weakness persists, movement keeps shrinking, night pain continues, or everyday activities are becoming harder.

Questions for your clinician

Ask whether your symptoms come mainly from the tendon tear, the joint surface, or both; which examination findings support that view; whether imaging would guide treatment; and which movements are safe while you rebuild function. You can also ask what improvement would be expected from physiotherapy and what findings would make a surgical opinion useful.

Questions people ask

Does every rotator cuff tear lead to shoulder osteoarthritis?

No. Many tears do not progress to joint arthritis. Risk depends on features such as tear size, tendon and muscle function, shoulder mechanics and how the condition changes over time.

Can physiotherapy repair the torn tendon or worn cartilage?

Physiotherapy does not reattach a fully torn tendon or regrow worn cartilage, but it may improve movement, muscle coordination, comfort and the ability to perform daily tasks.

Can rotator cuff tear cause shoulder osteoarthritis?

A substantial or long-standing tear can contribute by allowing abnormal movement and loading inside the shoulder, although a tear does not inevitably cause arthritis.
