Can Perthes disease cause hip osteoarthritis?

Perthes disease disrupts blood supply to the femoral head during childhood. As the bone heals and remodels, the ball of the hip may become less round or fit the socket less evenly. That altered fit can concentrate pressure on part of the joint surface and contribute to cartilage wear in adulthood.

A round femoral head that sits well inside the socket spreads load more evenly. Flattening, enlargement, or reduced coverage can change hip mechanics, restrict rotation, and allow the ball and socket to make contact unevenly. Osteoarthritis is a possible later effect, not an inevitable outcome for every person who had Perthes disease.

Hip osteoarthritis may present with deep groin or buttock pain, stiffness after rest, reduced walking tolerance, or difficulty reaching shoes and socks. Pain can also be felt in the thigh or knee. These symptoms have several possible causes, so the childhood history helps the clinician interpret the examination and imaging rather than confirming the diagnosis by itself.

Book a musculoskeletal assessment if pain or stiffness persists, progressively limits walking, disturbs sleep, or changes how you move. Tell the clinician which hip was affected, what childhood treatment you received, and whether older images or operation details are available. Examination may assess gait, hip motion, strength, and whether pain could be coming from the back or another structure.

Treatment is based on current symptoms and joint structure. Options can include activity adjustment, strengthening and mobility work, suitable pain relief, and a walking aid used on the opposite side when advised. An orthopaedic opinion may be useful when deformity or arthritis substantially limits daily life; possible procedures depend on the remaining cartilage and the shape of both the ball and socket.

Seek urgent care after a fall if you cannot stand or bear weight, the leg looks shortened or turned, or the pain is severe. A hot, swollen joint with fever or feeling acutely unwell also needs urgent assessment because infection must be considered rather than attributed to old Perthes disease or osteoarthritis.

Arrange an earlier clinic review if pain escalates quickly, night pain is new and persistent, or function declines despite the current plan. Gradually increasing stiffness without emergency signs still deserves assessment when it interferes with work, self-care, sleep, or walking, because management can be tailored before those limits become more disruptive.

Consider asking whether your current symptoms arise from osteoarthritis, impingement, tendon irritation, or another source. Ask what the images show about joint space and femoral head shape, which exercises protect function without provoking symptoms, whether a walking aid is appropriate, and what change would trigger referral to a hip specialist.
