Can pelvic inflammatory disease cause hydrosalpinx?

The affected tube may become swollen and unable to transport an egg normally. Hydrosalpinx can involve one tube or both, and it is sometimes discovered during fertility assessment rather than because of symptoms.

PID can inflame the inner lining of a fallopian tube and surrounding pelvic tissue. As healing occurs, scar tissue may close the far end of the tube. Normal tubal secretions then become trapped behind the closure, stretching the tube into a fluid-filled swelling.

A hydrosalpinx may reduce the chance of pregnancy by blocking egg and sperm transport. Fluid can also pass back toward the uterus and interfere with implantation. The condition may cause pelvic discomfort, but the absence of pain does not show that the tubes are unaffected.

A gynaecologist may use pelvic ultrasound or a test that follows fluid or contrast through the uterus and tubes. The most useful test depends on whether the concern is pain, a previous infection or difficulty becoming pregnant.

Antibiotics are used for active infection, but they do not drain a longstanding hydrosalpinx or remove established scarring. If pregnancy is the goal, a fertility specialist may discuss monitoring, surgery on the affected tube or assisted conception. The choice depends on symptoms, the other tube, ovarian factors and your preferences.

Contact a clinician promptly for new pelvic pain, fever, unusual discharge, bleeding between periods or pain during sex, as these can accompany pelvic infection. Seek urgent care for severe pain, fainting, repeated vomiting or heavy bleeding. If pregnancy is possible, pain or bleeding needs rapid assessment to exclude an ectopic pregnancy.

Ask how confident the imaging is, whether one tube or both appear affected, and whether there are signs of active infection. If you hope to conceive, ask how the hydrosalpinx may influence your options and what benefits and downsides each procedure carries.
