Can pelvic inflammatory disease cause ectopic pregnancy?

This link does not mean that every person with previous PID will have an ectopic pregnancy. It does mean that a clinician should know about your PID history when you become pregnant or are planning pregnancy.

After fertilisation, the early pregnancy normally travels through a fallopian tube to the uterine cavity. Inflammation from PID can damage the tube's lining, narrow its channel or create adhesions. These changes can delay that journey and allow implantation within the tube.

A tubal pregnancy cannot develop safely in the narrow tube and can cause internal bleeding if the tube ruptures. Symptoms alone cannot establish where a pregnancy is located, so diagnosis may involve pregnancy hormone blood tests and transvaginal ultrasound.

Tell your maternity clinician about any previous PID, sexually transmitted infection, ectopic pregnancy or tubal surgery. After a positive pregnancy test, ask when an early review is appropriate. The timing of imaging depends on symptoms, menstrual dates and blood-test findings.

If you currently have pelvic pain, unusual discharge, bleeding after sex or fever, seek assessment for possible infection. Prompt antibiotic treatment can stop ongoing infection, although it cannot reliably reverse scar tissue that is already present.

Seek urgent medical care if pregnancy is possible and you have one-sided lower abdominal pain, vaginal bleeding or brown discharge, shoulder-tip pain, dizziness, fainting, marked weakness or sudden severe abdominal pain. Do not wait for a routine appointment, as internal bleeding can become dangerous quickly.

Ask when the pregnancy location can be checked, which symptoms require emergency care, and whether serial blood tests or an ultrasound are indicated. If an ectopic pregnancy is diagnosed, ask how the proposed treatment works and what follow-up confirms that treatment is complete.
