Stress fracture and insufficiency fracture

A traditional fatigue stress fracture develops when otherwise healthy bone is repeatedly loaded beyond its capacity to repair. An insufficiency fracture follows ordinary loading of bone that has reduced strength. Osteoporosis is one reason that bone may be vulnerable, although other bone and metabolic conditions can contribute.

An insufficiency fracture can present with focused pain that worsens during standing, walking or another repeated activity and eases with rest. Swelling or tenderness may occur, but a major fall is not required. Commonly affected weight-bearing areas include the pelvis, sacrum, hip region, shin and foot.

How to respond to persistent focal pain

Pause running, jumping and other activity that reproduces the pain. Reduce loading and arrange a clinical assessment rather than repeatedly testing the sore area. The clinician will examine the location and may request imaging; an early fracture is not always visible on the first plain X-ray, so further imaging may be considered when suspicion remains.

Management depends on the bone involved and whether the fracture is stable. It may include activity modification, protected weight-bearing, a brace or walking aid, and gradual rehabilitation. Assessment for osteoporosis and other causes of weak bone can guide nutrition, medication and a safer return to movement.

When pain needs prompt or urgent care

Arrange prompt care for pain in one precise bone area that persists, increases with weight-bearing, causes a limp or follows a recent rise in activity. Seek urgent assessment if you suddenly cannot bear weight, pain becomes severe, a limb looks deformed, or the area is accompanied by marked redness, heat, fever, numbness or a cold, pale foot.

Ask whether the pattern fits a fatigue or insufficiency fracture, whether additional imaging is needed if an X-ray is normal, how much weight you may place on the area, and what signs should trigger urgent reassessment. Clarify whether bone-density testing or tests for secondary causes of weak bone are appropriate.
