Can lymphoma cause superior vena cava obstruction?

Restricted blood flow can cause swelling of the face, neck, arms, or upper chest. Breathlessness, cough, headache, a feeling of fullness in the head, and prominent veins across the chest may also occur.

The superior vena cava carries blood from the head, neck, arms, and upper chest back to the heart. Lymphoma within the chest can narrow the vein from outside, and impaired flow makes blood return through smaller veins instead.

Symptoms may become more noticeable when bending forward or lying flat. A clinician may arrange chest imaging to locate the narrowing and assess nearby lymph nodes, then use tissue testing to confirm the lymphoma type if it is not already known.

New facial or neck swelling, increasingly visible chest veins, or worsening breathlessness should be reported promptly to the cancer team or assessed in an urgent medical service. Sit upright if lying flat makes breathing or head pressure worse.

Management is tailored to the severity of the blockage and its cause. Treatment directed at lymphoma can shrink the tissue pressing on the vein. Some people need a procedure to support the narrowed vein, while medicines may be used for particular symptoms or complications.

Call emergency services for severe breathing difficulty, noisy breathing, fainting, confusion, marked drowsiness, a blue or grey colour around the lips, or rapidly increasing swelling. These can indicate that breathing or blood flow is being seriously affected.

Do not drive yourself if you feel faint or cannot breathe comfortably. Tell the emergency team about known or suspected lymphoma and any central venous line, because these details can guide immediate assessment.

Ask what is narrowing the vein, whether the airway is also affected, which treatment should start first, what improvement to watch for, and which changes mean you should go directly to an emergency department.
