Can Li-Fraumeni syndrome cause soft tissue sarcoma?

The TP53 gene normally helps prevent damaged cells from continuing to divide. A disease-causing variant present from birth reduces that protection in tissues throughout the body. This can create vulnerability to several cancers, including sarcomas that begin in muscle, fat, blood vessels or other supporting tissues.

Soft tissue sarcoma is a group of cancers rather than one disease; liposarcoma and leiomyosarcoma are examples. The type, body site and age at diagnosis can vary within Li-Fraumeni families. Genetic susceptibility cannot predict that a particular person will develop a particular sarcoma.

Follow the personalised surveillance programme from your specialist genetics or oncology service. It may include regular examination and imaging that avoids ionising radiation where clinically appropriate. Keep the coordinating team informed about new diagnoses in close relatives because family history may shape individual advice.

Check new lumps without repeatedly squeezing them. Request an appointment for a lump that is growing, feels deep or firm, becomes painful, or returns after removal. Also report unexplained swelling, reduced movement or persistent discomfort in one area. Most lumps are not sarcoma, but their features determine whether imaging or biopsy is needed.

Notify the surveillance team promptly about a rapidly enlarging mass, new weakness or numbness near a lump, or swelling that interferes with walking or using an arm. A clinician can document its location, depth and mobility and arrange the most suitable test.

Obtain urgent care if swelling suddenly compromises breathing, causes severe uncontrolled pain, or is accompanied by a cold, pale or weak limb. Those problems require immediate evaluation regardless of their cause and should not wait for a genetics clinic visit.

Discuss how to contact the surveillance team between appointments, what examination schedule applies to you, and whether a newly noticed lump should be assessed locally or at a sarcoma service. Ask the genetics clinician which relatives may benefit from counselling about the known family variant.
