This connection does not mean that every person with impingement will develop osteoarthritis. Joint shape, existing cartilage or labral damage, previous injuries, activity demands and other individual factors all influence what happens over time.

What happens inside an impinging hip

Hip impingement involves contact between the top of the thigh bone and the rim of the hip socket during certain movements. Repeated contact can irritate the labrum and damage the smooth cartilage that helps the joint glide. Osteoarthritis develops when joint cartilage and the underlying joint structures change and no longer move as smoothly.

Impingement often causes groin discomfort with deep bending, twisting or prolonged sitting. Hip osteoarthritis can also cause stiffness and activity-related pain. Because the symptom patterns overlap, an examination and appropriate imaging are used to distinguish the likely sources.

Reduce irritating loads while staying active

Temporarily modify movements that repeatedly pinch the front of the hip, such as very deep squats or forceful twisting. Choose comfortable ranges rather than stopping all movement. A physiotherapist can assess hip and trunk control, then tailor strengthening and mobility work to the activities you need.

Persistent symptoms deserve a clinical assessment. Management may include activity changes, rehabilitation and pain-relief options; selected structural problems may need an orthopaedic opinion. Treatment is based on symptoms, examination findings, imaging and functional goals rather than an imaging label alone.

Signs that should change the timing of care

Arrange an assessment if groin pain keeps returning, stiffness is increasing, the hip catches or locks, or everyday tasks are becoming difficult. Earlier review can identify whether symptoms fit impingement, osteoarthritis or another hip problem.

Seek urgent care for severe pain after a fall, inability to bear weight, an obviously deformed leg, a hot swollen joint with fever, or new weakness or numbness. These features can indicate an acute injury, infection or nerve problem rather than routine impingement.

Make the consultation practical

Ask which movements reproduce the joint contact, whether the labrum or cartilage appears affected, and what activity changes are sensible for work or sport. Clarify what improvement should look like during rehabilitation and which findings would justify further imaging or a surgical opinion.
