The short answer

The cause cannot be identified from the burning quality alone. Movement-related pain after sport, pain with a groin bulge, and pain accompanied by urinary or scrotal changes require different examinations. Sudden testicular pain is an emergency even when the discomfort is also felt in the groin or lower abdomen.

How to interpret the pattern

Pain beginning during sprinting, kicking or rapid direction changes may involve an adductor or another groin muscle. Deep discomfort provoked by hip flexion and rotation can occur when hip impingement or a hip labral tear presents with groin pain. Hip osteoarthritis can present with stiffness and pain affecting walking, footwear or transfers.

A bulge that appears with coughing or straining raises concern about a hernia. Burning during urination, discharge, blood in urine, fever, scrotal swelling, a rash, or tingling changes the clinical picture. Symptoms travelling from the back or accompanied by altered skin sensation can indicate nerve involvement.

Managing symptoms safely

Stop the sporting movement or heavy lifting that triggered the pain and avoid repeatedly checking it with forceful stretches. Continue easy, pain-limited movement if walking is comfortable. Do not attempt strenuous exercise when there is a new lump, testicular discomfort, fever or urinary symptoms.

Schedule an assessment for persistent or recurrent symptoms, a change in walking, pain during daily tasks, or unexplained groin burning. Record the exact site, onset, triggers, back pain, sensory changes and whether a lump changes with coughing. Mention urinary, sexual and scrotal symptoms directly; they can determine which examination or test is needed.

When care is urgent

Go to emergency care for sudden or severe testicular pain, especially with swelling, nausea, vomiting or a testicle sitting differently. Do not wait to see whether it settles. A very painful groin lump that becomes firm or cannot be reduced, particularly with abdominal pain or vomiting, also needs emergency evaluation.

Obtain prompt help for fever with worsening groin or scrotal redness, inability to pass urine, blood in urine, major trauma, inability to bear weight, new leg weakness, numbness around the genitals, or loss of bladder or bowel control.
