What the lack of itch tells you
No itch does not identify one diagnosis. Smooth or rough bumps may occur with keratosis pilaris, irritation, contact dermatitis or psoriasis. Lichen Planus can produce firm, flat-topped bumps, while a yeast infection may affect warm, moist folds. Some conditions, including Atopic Dermatitis or Scabies, often itch, but symptoms vary and absence of itch does not rule out every possibility.
Notice whether the bumps are skin-coloured or red, flat or domed, grouped or scattered, and whether they are tender, scaly, moist or centred on hairs. Compare the area with nearby skin and note new products, medicines, illness, travel or friction.
A sensible first approach
Clean gently with lukewarm water, moisturise with a fragrance-free product and avoid picking, squeezing and unneeded medicated creams. Keep folds dry and reduce rubbing from tight clothing. Photograph the area in consistent light so you can notice change. Do not treat a persistent or uncertain bump with home freezing or leftover antibiotics; a clinician may need to distinguish eczema, psoriasis, infection or a separate lesion before choosing treatment.
When to seek medical care
Arrange an appointment if bumps persist, multiply, recur, become painful, bleed, crust, ulcerate, or change in size, shape or colour. Seek urgent help for rapidly spreading redness, fever, facial or tongue swelling, breathing difficulty, severe pain, or involvement around the eye. Bring a list of medicines and products, and mention whether anyone close to you has a similar rash.
