A dental or medical examination is needed if the bump persists, grows, feels firm, ulcerates or is accompanied by other mouth or neck changes. Its appearance cannot confirm or exclude cancer.
Possible explanations
A hard midline prominence may be a harmless bony growth called a torus palatinus. Other causes include irritation from hot food or dental appliances, a blocked minor salivary gland, infection, a cyst or another benign growth.
A concerning lesion may be firm or enlarging, fail to heal, bleed, develop red or white surface changes, or cause numbness. Mouth cancer can also present with persistent pain, swallowing difficulty, altered speech, loose teeth or a lump in the neck.
How to get it checked
Arrange an appointment with a dentist, oral medicine clinician or doctor for a bump that does not settle after a short period, has no clear explanation or is changing. They will inspect and feel the roof of the mouth and check the rest of the mouth and neck.
Note when you first saw it, whether it changes in size, and whether eating or dentures irritate it. Avoid picking or attempting to drain the area. If examination does not explain the lesion, referral for imaging or a small tissue sample may follow.
When care is urgent
Seek urgent assessment if swelling is rapidly increasing, fever accompanies spreading facial swelling, or pain prevents drinking. Call emergency services if swelling interferes with breathing or swallowing, or if bleeding is heavy and does not stop.
Prompt review is also warranted for a persistent ulcer, unexplained numbness, a new neck lump or progressive difficulty swallowing.

Ask what features suggest the likely cause, whether the nearby teeth or salivary glands are involved, whether imaging or biopsy is appropriate, and how soon the area should be reviewed if it remains unchanged.

Is a hard bump in the middle of the palate always abnormal?
No. A torus palatinus is a benign bony prominence that can occur along the midline, but a new or changing lump should still be examined.
Will a clinician need to take a biopsy?
Not every bump needs one. A biopsy may be recommended when examination and imaging do not adequately explain the lesion or its features are concerning.
