Broken wrist rehabilitation exercises: a safe return to movement

Short answer
Broken wrist rehabilitation should progress from permitted finger and arm movement to guided wrist mobility and strengthening only when the fracture is ready.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Early focus
- Maintain permitted movement in joints not immobilised
- Later focus
- Recover wrist motion before gradually increasing strength and load
- Useful feedback
- Pain, swelling and function after each activity
Why the exercise plan changes over time
Bone stability, treatment type, swelling and stiffness determine which movements are appropriate. While the wrist is immobilised, the plan may focus on moving exposed fingers, the elbow and shoulder. Wrist exercises generally wait until the treating clinician confirms that movement is suitable.
After immobilisation, the wrist can feel weak and restricted. Rehabilitation restores motion gradually, then adds grip and load as healing and symptoms allow. A hand therapist can adapt the programme when surgery, joint stiffness, nerve symptoms or complex work demands affect recovery.
How to exercise without overloading the wrist
Use only exercises cleared for your current stage. Move slowly within the advised range, keep the forearm supported when helpful, and avoid forcing through sharp pain. Common later movements include bending the wrist forward and back, turning the palm up and down, and gentle side-to-side motion.
Strength work may begin with light gripping or resistance prescribed by the therapist. Record which activity causes swelling or pain and how long the reaction lasts; that pattern helps the clinician adjust repetitions, resistance and rest.
Stop and seek clinical advice for warning symptoms
Stop the exercise and seek prompt assessment for a new deformity, sudden severe pain, a fresh fall, loss of finger movement, or persistent numbness. Fingers that turn pale or blue, feel cold, or develop rapidly worsening swelling require urgent evaluation.
A mild temporary ache can occur as movement returns, but pain that repeatedly escalates after the programme deserves review. Redness, wound discharge or fever after an operation also needs medical attention rather than further exercise.
Questions that shape your programme
Confirm which movements are allowed now, whether a splint should come off for practice, and what level of discomfort is acceptable. Ask when resistance, lifting, driving, sport and work tasks can be introduced, and whether hand therapy is appropriate for your goals.

Questions people ask
Should wrist exercises hurt after a fracture?
Gentle stiffness or a mild ache may occur, but sharp, severe or progressively worsening pain is a reason to stop and contact the treating clinician or therapist.
Can I exercise while my wrist is still in a cast?
Only perform movements approved by your treating team. Finger, elbow or shoulder motion may be encouraged, but do not move or load the immobilised wrist unless specifically instructed.
Why is my wrist weak after the cast comes off?
Immobilisation limits normal movement and muscle use. Strength usually needs to be rebuilt gradually after the fracture is stable enough for increasing resistance.
