What persistent swelling may mean

Fluid may collect because injured tissues remain inflamed and the calf and ankle are moving less than usual. A clinician should reassess swelling that no longer responds to rest and elevation or is accompanied by new symptoms.

Look at the pattern, not only the size

Swelling that increases after being upright and eases overnight can occur during fracture recovery. A steady increase may instead relate to excessive loading, irritation from a cast or boot, a new injury, infection, or a problem with blood flow.

Warmth and redness spreading beyond the injured area, drainage, fever, new calf tenderness, or a marked change in pain makes the pattern more concerning. Persistent stiffness can also limit the muscle-pump action that normally helps move fluid away from the foot.

Steps to take while arranging review

Reduce activities that clearly aggravate the swelling and follow the prescribed weight-bearing limit. Support the whole lower leg while elevating it, keep the heel free from pressure, and gently move the toes. Use only exercises already approved for your current stage.

Check that straps are positioned as instructed, but do not reshape a cast or change a boot setting without guidance. Record when swelling is greatest and whether elevation changes it. Tell the clinical team about any recent fall, increased walking, missed follow-up or damage to the support.

Signs that should not wait

Get prompt advice if swelling remains pronounced despite elevation, the cast or boot is painfully tight, pain is intensifying, or a wound develops discharge or an unpleasant smell. These findings may require the support and skin to be checked.

Seek emergency care for a cold, pale or blue foot; new numbness; inability to move the toes; severe pain; sudden breathlessness; chest pain; or painful swelling extending into the calf. Do not massage a newly painful, swollen calf.

What to ask at review

Ask whether the fracture position and soft tissues need reassessment, whether your support still fits, and if your current activity matches the healing stage. Confirm which exercises can restore movement safely and how you should modify walking when swelling increases.
