Because the infection can be walled off, symptoms may be less dramatic than those of an acute infection. Persistent focal bone pain still deserves evaluation even when there is no obvious wound or high fever.

The body forms a rim of reactive bone around an infected cavity. This can limit spread but can also make it difficult for antibiotics to reach all infected or unhealthy tissue.

Pain may be localised, deep, and recurrent, sometimes becoming more noticeable at night. Examination, blood tests, X-rays, and more detailed imaging can help define the lesion, although a tissue or fluid sample may be needed to identify the organism and exclude conditions that resemble infection.

Arrange an orthopaedic review for persistent focal bone pain, especially after a previous infection, operation, penetrating injury, or unexplained swelling. Avoid repeatedly treating the pain without establishing its cause.

Treatment may include opening and cleaning the cavity, taking cultures, and giving antibiotics selected for the likely or confirmed organism. If cleaning leaves a meaningful bone defect, the surgeon may discuss ways to fill or reconstruct it. Follow-up checks whether symptoms settle and the bone recovers.

Seek prompt medical care if pain rapidly worsens, redness spreads, swelling increases, pus appears, or fever and chills develop. New inability to use the limb or bear weight also warrants urgent assessment.

After a procedure, report wound leakage, wound opening, increasing heat, or worsening pain rather than waiting for a routine visit. Call emergency services for confusion, collapse, severe breathlessness, or a rapidly deteriorating general condition.

Ask what the images show, whether a biopsy or culture is required, how antibiotic choice will be made, whether surgery could weaken the bone temporarily, and how activity should change during recovery.
