What the colour can mean

Fresh blood is often seen on toilet paper, on the outside of stool, or in the toilet water. The bleeding may come from the anus or lower bowel, but colour alone cannot identify the source. A condition such as ulcerative colitis or Crohn's Disease may present with rectal bleeding alongside diarrhoea, urgency, cramps, or mucus. Diverticular Disease can also cause bleeding. Less commonly, bowel growths including Bowel Cancer or Rectal Cancer need to be considered, especially when bleeding recurs or bowel habits change.

What to do next

Note when the bleeding happens, how much you see, whether it is mixed into the stool, and whether pain, constipation, diarrhoea, fever, dizziness, or weight change is present. Arrange a clinical assessment for repeated, unexplained, or first-time bleeding. Examination may involve looking at the anal area and discussing bowel symptoms; further tests depend on your history. Do not stop prescribed medicines without advice. If a polyp is found, Polyp Removal may be discussed as a treatment.

When to seek urgent care

Seek urgent medical help for heavy or ongoing bleeding, black or tar-like stool, faintness, confusion, severe abdominal pain, vomiting blood, or marked weakness. Prompt assessment is also needed if bleeding occurs with fever, a swollen abdomen, or inability to pass stool or gas. These warning signs matter regardless of whether the bleeding is bright red or only noticed once.
