Act promptly on a suspected abscess

Do not squeeze, massage, puncture, or try to drain the swelling at home. Keep the area clean and avoid applying oils, creams, herbs, or leftover medicines unless the baby's clinician specifically recommends them.

Normal newborn swelling versus infection

Newborns can develop temporary breast tissue swelling from hormones transferred before birth. This may affect one or both sides and can occasionally produce a small milky discharge. That normal change is different from mastitis, an inflamed or infected breast, and from an abscess, which is a pocket of pus within infected tissue.

Infection may present with increasing redness, warmth, tenderness, firmness, swelling, or pus. A baby may also feed less, become unusually sleepy or irritable, or develop an abnormal temperature. A clinician must examine the baby because early appearances can overlap and newborn infections can worsen quickly.

What assessment and treatment may involve

Contact the baby's pediatrician or an urgent pediatric service now and describe the baby's age, temperature, feeding, alertness, wet nappies, when the swelling began, and whether redness is spreading or fluid is draining. If the baby is unwell, go directly to emergency care.

The clinician may use examination and a breast ultrasound to look for a fluid collection. Antibiotics are selected for the baby's age and condition. If pus has collected, breast abscess drainage may be needed by needle aspiration or a small incision; any sample can help guide antibiotic choice. Follow-up checks that redness, swelling, and feeding are improving.

Emergency warning signs in a newborn

Seek emergency care immediately if the baby is difficult to wake, feeds poorly or refuses feeds, has breathing trouble, looks pale or mottled, has fewer wet nappies, develops an abnormal temperature, or if redness spreads quickly. Pus, rapidly increasing swelling, or a baby who seems generally unwell also warrants immediate assessment.
