Breakthrough bleeding after years on the pill

A missed or late pill, vomiting, diarrhoea, a newly started medicine, or a change in how the pill is taken may destabilize the uterine lining. Pregnancy, cervical irritation, infection, and uterine conditions can also cause unexpected bleeding, so long-term trouble-free use does not identify the cause.

Review recent doses before assuming the pill has stopped working. Timing errors and stomach illness can affect hormone exposure, while some medicines and herbal products can interfere with certain pills. Taking active pills continuously may also allow spotting even after a long stable interval.

Chlamydia may present with bleeding between periods or after sex, sometimes with discharge or pelvic pain. Endometrial polyps may present with irregular bleeding. These conditions require appropriate testing or examination; the appearance of the blood cannot distinguish them.

Continue taking the pill according to its instructions unless a clinician advises otherwise. Check the packet guidance for missed doses, vomiting, diarrhoea, and backup contraception. Ask a pharmacist whether any new prescription, over-the-counter product, or supplement interacts with your pill.

Take a pregnancy test if doses were missed, absorption may have been reduced, or pregnancy symptoms are present. Log spotting, pill timing, sex-related bleeding, pain, and discharge. Arrange an appointment if the change repeats, persists, or disrupts daily life; cervical screening should also be kept up to date when eligible.

Urgent assessment is needed for heavy bleeding with dizziness or fainting, severe pelvic or abdominal pain, breathlessness, or marked weakness. If pregnancy is possible, bleeding with one-sided pain, shoulder-tip pain, or collapse is an emergency warning pattern.

Seek prompt care for fever, foul-smelling discharge, escalating pelvic pain, or bleeding after menopause. Mention new headaches, visual symptoms, chest pain, leg swelling, or sudden shortness of breath urgently because combined hormonal pills may be unsuitable while those symptoms are assessed.

Bring the pill name and a list of all medicines and supplements. Ask whether the dosing schedule could explain the bleeding, whether pregnancy or infection tests are indicated, whether a pelvic or cervical examination is appropriate, and whether a different pill or contraceptive method would suit you better.
