Breaking the water to induce labor

It may be offered to start or strengthen labour, sometimes alongside medicine that stimulates contractions. It is not suitable in every pregnancy, and consent should follow a discussion of the reason, alternatives, expected sensations, and possible complications.

During a vaginal examination, a trained clinician passes a small sterile instrument through the open cervix and makes an opening in the membranes. You may feel examination pressure and then warm fluid draining. The baby remains inside the uterus; only the surrounding membranes are opened.

Before proceeding, the team checks factors such as cervical opening, the presenting part of the baby, and heartbeat. Monitoring before and after rupture helps identify a heartbeat change or a rare cord problem promptly.

Tell the team about previous births or uterine surgery, medicines, allergies, bleeding, unusual discharge, and recent changes in the baby's movements. Ask for an explanation before the examination and say if you need it paused. You can request privacy measures, a support person where permitted, and help with pain or anxiety.

Once the membranes are open, they cannot be resealed. Clarify what the next step would be if contractions remain weak, how movement and comfort will be supported, and when another delivery plan might become advisable.

While in the maternity unit, alert staff immediately to heavy bleeding, severe constant pain between contractions, breathing difficulty, faintness, feverishness, or a sudden change in the baby's movements. Staff also need to know if you feel something in the vagina or have a strong urge to push.

If fluid had already begun leaking at home before a planned induction, call the maternity unit before travelling in. Ruptured membranes can be a presentation of preterm labour when they occur before term, so pregnancy stage changes the urgency and care pathway.

Consider asking why amniotomy is recommended now, whether the cervix and baby's position make it suitable, and what monitoring will be used. You can also ask about alternatives, pain support, freedom to move, medicine that may follow, and circumstances that would change the birth plan.
