Breaking the waters during induction

It is called artificial rupture of membranes or amniotomy. A midwife or doctor makes a small opening in the amniotic sac during a vaginal examination so fluid can drain.

Amniotomy is considered when the cervix is open enough to reach the membranes and the baby's head is in a suitable position. It may be offered after medicine or a balloon has helped prepare the cervix, or as part of a planned induction sequence.

Releasing the fluid may help contractions begin or become more effective, but it does not guarantee that labour will progress. An oxytocin drip may be discussed if contractions do not establish, depending on your circumstances and maternity plan.

The baby's heart rate is usually checked around the procedure. You may feel the vaginal examination, pressure, warmth, and a gush or trickle of fluid; the opening itself does not cut your skin. Fluid can continue leaking, so use the pad provided and tell staff if its colour or smell changes.

Before agreeing, ask why amniotomy is recommended now, what alternatives remain, how discomfort can be managed, and what the next step would be if contractions do not start. You can request an explanation before or during the examination and ask the clinician to pause.

Tell staff immediately about fresh bleeding, severe constant pain, feeling faint or feverish, breathing difficulty, a sudden urge to push, or reduced baby movements. Changes in the baby's heart rate or a concern that the cord has slipped below the baby require urgent clinical action.

If your waters break before a planned induction or while you are at home, contact your maternity unit for guidance. Note the time, fluid colour and smell, wear a pad rather than inserting anything into the vagina, and report bleeding, fever, pain, or reduced movements without delay.

Useful questions include whether the baby's head is well positioned, how the heart rate will be monitored, when progress will be reassessed, and whether oxytocin is likely to be offered. Ask how breaking the waters may affect movement, bathing, examinations, and your pain-relief options in that maternity unit.
