What the tightening may mean

Braxton Hicks are practice tightenings of the womb. They may feel like the abdomen briefly becomes firm, then relaxes. They are often irregular and may settle after rest, changing position, emptying your bladder, or drinking water. At 20 weeks, repeated tightening deserves more caution because true preterm labour can begin with cramps, backache, pelvic pressure, or a change in vaginal discharge. A tightening episode alone cannot confirm the cause.

What to do now

Stop strenuous activity and note when each tightening starts, how long it lasts, and whether the pattern changes. Rest on your side, drink fluids if you have not been drinking normally, and avoid trying to manage persistent contractions with medication unless your maternity team has advised it. If the sensation eases and does not return, mention it at your next appointment; if it keeps recurring, call your maternity unit for individual guidance.

When to seek care

Contact your maternity unit promptly for regular or increasingly strong tightenings, period-like pain, lower back pain, pelvic pressure, vaginal bleeding, watery fluid, unusual discharge, fever, or reduced fetal movement if you have already started feeling a pattern. Seek emergency help for heavy bleeding, severe constant pain, fainting, or feeling seriously unwell. Do not wait for symptoms to become severe before asking for advice at this stage of pregnancy.
