What it means

Malaria is an infection transmitted by infected mosquitoes. In severe disease, infected red blood cells and inflammation can disrupt blood flow and the brain’s normal function. Cerebral malaria is diagnosed from the overall clinical picture alongside evidence of malaria; it is not simply a severe headache or fever.

Early illness may include fever, chills, sweating, headache, marked tiredness, or muscle aches. Brain involvement is suggested by confusion, unusual behaviour, difficulty staying awake, inability to respond normally, weakness, or seizures. Symptoms can worsen quickly, especially in children, pregnant people, older adults, and people with limited immunity.

What to do

Treat suspected cerebral malaria as an emergency. Call local emergency services or go to an emergency department immediately, particularly if a person with fever after travel or residence in a malaria area becomes confused, drowsy, difficult to wake, or has a seizure. Do not wait for symptoms to settle and do not rely on home remedies.

Tell the medical team where the person has been, when symptoms began, and whether malaria prevention medicine was taken. Clinicians may perform urgent blood tests and monitor breathing, blood sugar, temperature, fluid balance, and neurological function. Hospital treatment uses antimalarial medicine and supportive care; the exact plan depends on severity and local protocols.

When to seek care

Seek emergency care now for any altered mental state, seizure, collapse, severe breathing difficulty, repeated vomiting with weakness, or inability to drink in someone who may have malaria. A fever after travel to a malaria-risk area also needs prompt medical assessment, even if the person initially feels alert.

A negative early test may not settle every case if symptoms continue, so follow the treating clinician’s advice about repeat testing and observation. Untreated severe malaria can damage several organs, which is why neurological symptoms should never be watched at home.
