How to interpret symptoms around HRT
HRT can be used for menopausal symptoms, and concentration may change as other symptoms and sleep change. A new patch, gel, tablet or dose can also coincide with headaches, mood changes or other effects that need discussion. Brain fog is not a reliable way to judge whether HRT is working, and it does not prove that estrogen is too low or too high. Depression, anxiety, burnout, insomnia, iron deficiency anaemia and obstructive sleep apnoea can look similar. The timing of symptoms in relation to treatment is especially helpful.
Keep treatment decisions safe
Record the product, dose, start or change date, symptom pattern, sleep and hot flushes. Use reminders and written notes for tasks while you are foggy, and keep a regular sleep routine. Do not stop, double or switch HRT without advice; contact the prescriber if symptoms began after a change or are troubling. A review can check how the treatment is being used, consider interactions and assess whether another explanation or a different approach deserves attention.
When to get help quickly
Book a review if brain fog affects employment, driving, self-care or medicines, or if it continues despite other menopausal symptoms settling. Seek urgent help for sudden confusion, difficulty speaking, one-sided weakness or numbness, seizure, fainting, chest pain or severe breathlessness. Report concerning bleeding or a severe new headache promptly as well.

Ask whether the timing suggests a treatment effect, what benefit to expect, and whether sleep, mood, blood tests or another medicine should be reviewed. Clarify what to do if a dose is missed and which warning symptoms require urgent care.
