Bow legs treatment without surgery

Short answer

Non-surgical care for bow legs depends on age, cause, severity, symptoms and whether the alignment is changing.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Clinical term
Bow legs are also called genu varum.
Exercise
It can improve function and control but may not change structural bone alignment.
Monitoring
Age, symmetry and change over time help guide care.

Can bow legs be treated without surgery?

Young children with a typical developmental leg shape may only need observation and scheduled checks. Exercise can improve strength, balance and movement control, but it does not reliably reshape a bone that is structurally curved. Bracing or treatment of an underlying nutritional or medical problem is appropriate only when a clinician identifies a specific reason.

What non-surgical care can achieve

The aim is matched to the cause. Monitoring records whether alignment improves, remains stable or becomes more pronounced as growth continues. Physiotherapy may address weak hip or leg muscles, stiffness, an altered walking pattern and difficulty with daily activity. Insoles can sometimes improve comfort when foot position contributes to symptoms, but they do not straighten the thighbone or shinbone.

A clinician may examine standing alignment, knee motion, foot position and the way weight passes through each leg. Imaging or blood tests may be considered when the pattern is unusual, one side differs from the other, or an underlying bone condition is suspected.

Practical next steps

Arrange an assessment before buying braces or corrective devices. Note when the bowing began, whether it is changing, and whether pain, limping or frequent tripping occurs. Bring previous images or clinic notes if available. Follow an individual exercise programme consistently and use footwear that is comfortable and stable.

For a child, photographs taken occasionally in the same standing position can help a clinician compare visible change. Avoid forcing the knees together or using unsupervised stretching to try to alter bone alignment.

When to seek care

Book a timely orthopaedic review if bowing is worsening, clearly affects only one leg, begins after an injury, or is accompanied by pain, a persistent limp, reduced activity or difficulty walking. A child who is not growing as expected or whose leg shape looks increasingly uneven also needs medical assessment.

Seek urgent care after a significant injury if the person cannot bear weight, the leg looks newly deformed, or there is severe swelling, numbness, weakness, or a cold or pale foot.

Questions for your doctor

Ask whether the shape is coming from the hip, thighbone, knee, shinbone or foot; whether it is expected to change with growth; and how progress will be measured. Clarify what each proposed exercise, brace or insert is intended to improve, and which changes should prompt an earlier follow-up.

Questions people ask

Can exercises straighten bow legs?

Exercises can improve muscle strength, flexibility, balance and walking mechanics. They are not expected to remodel a structurally bowed thighbone or shinbone.

Should every child with bow legs wear a brace?

No. A brace is used only for selected causes and stages of growth after clinical assessment; many developmental patterns are monitored instead.

When might surgery be discussed?

An orthopaedic surgeon may discuss it when significant structural deformity persists or progresses, causes symptoms, or places uneven stress across a joint despite appropriate conservative care.
