When only one leg looks bowed

Short answer
Bowing that is clearly greater in one leg deserves clinical assessment because asymmetry can reflect growth, injury or structural alignment differences.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main concern
- Clear asymmetry needs an explanation, especially if it progresses.
- Examination
- Both legs, gait, joint motion and limb length are compared.
- Treatment
- Chosen for the cause and functional effect.
Why one-sided bowing matters
The finding is not a diagnosis by itself. The person's age, when the difference appeared, whether it is progressing, and the presence of pain or limping help determine what evaluation is appropriate.
What the difference may represent
One leg can appear more bowed because of differences at the hip, thigh bone, knee, shin or foot. A previous fracture or growth-plate injury may affect alignment; rotation of the leg can also change its appearance when standing.
An orthopaedic examination compares limb length, knee direction, joint movement and foot position on both sides. Watching standing and walking helps distinguish a fixed shape from a movement pattern. Imaging is ordered when the history and examination suggest that bone alignment or growth requires measurement.
What you can do now
Arrange a non-urgent appointment if the asymmetry is persistent, even without pain. Bring details of prior leg injuries and note whether shoe wear, walking distance or sports ability has changed. Photos taken naturally over time may show progression, but avoid repeatedly posing or forcing the legs into position.
Treatment follows the cause rather than the appearance. Paediatric Physiotherapy may help strength, balance or gait when these are contributing factors. A marked structural difference or altered growth may require specialist monitoring, and Osteotomy is reserved for selected deformities after detailed planning.
When to seek care sooner
Prompt evaluation is warranted when the curve is visibly worsening, a limp develops, pain persists, the knee feels unstable, or everyday activity becomes limited. A child whose one-sided alignment changes during growth should be examined rather than simply compared with peers.
After an injury, seek urgent care for inability to stand on the leg, obvious new deformity, severe swelling or loss of feeling. Fever together with a hot, swollen joint also requires urgent medical attention.
Questions about the asymmetry
Ask which part of the limb creates the curve, whether leg lengths differ, and whether the alignment is fixed or changes with movement. Confirm how progression will be monitored and which functional change should prompt an earlier return.

Questions people ask
Can a previous injury make one leg bow?
Yes. Some fractures or injuries near a growth plate can alter alignment, so the clinician will ask about old trauma.
Is an X-ray always needed?
Not always. The history and physical examination guide whether imaging would add useful information about bone alignment or growth.
