Bottom Lip Swollen With a Cold Sore: What to Know

Cold sores are caused by a viral infection that commonly produces a tender, tingling area followed by fluid-filled blisters. Inflammation around the sore can make part of the lip look puffy. Swelling that spreads beyond the sore, appears suddenly after food or medicine, or comes with hives may fit angioedema, which can affect the lips. A sore inside the mouth, a cracked corner, dental irritation, or trauma can also produce local swelling. The appearance alone cannot identify the cause.

Keep the area clean and avoid picking, squeezing, or peeling the blister. Wash your hands after touching your mouth, and do not share lip products, utensils, or towels while the sore is active. A cool, clean compress may ease heat and puffiness. Avoid products that sting, including fragranced balms, and protect the sore from rubbing. A pharmacist or clinician can advise whether an antiviral treatment is suitable, especially when started early. Note any new medicine, food, toothpaste, or cosmetic used before the swelling began.

Seek emergency help immediately if lip swelling is accompanied by swelling of the tongue or throat, trouble breathing, wheezing, faintness, or difficulty swallowing. Arrange prompt medical assessment if swelling is rapidly increasing, the eye area is involved, fever or spreading redness develops, or you cannot drink. A clinician should also review a sore that is unusually extensive, keeps returning, or does not begin to settle.
