Understanding osteosarcoma of bone

It is different from cancer that began elsewhere and later spread to bone. Care is coordinated by a sarcoma team because diagnosis, biopsy planning, surgery and drug treatment need to be closely aligned.

Osteosarcoma can present with persistent pain, swelling, a lump or reduced movement near the affected bone. These symptoms also occur with injuries and non-cancerous conditions, so they require assessment rather than assumptions about their cause.

Imaging defines the bone abnormality and its relationship to joints, muscles, nerves and blood vessels. A carefully planned biopsy provides tissue for diagnosis. The biopsy route matters because it may later be removed with the tumour, which is why it should be arranged by the team that will perform definitive surgery. Chest and other imaging check whether disease is present elsewhere.

Chemotherapy may be given before surgery to treat cancer cells throughout the body and after surgery to continue systemic control. The surgeon aims to remove the tumour with an adequate rim of healthy tissue while preserving useful function when feasible. Reconstruction may use a metal implant, bone graft or another technique suited to the site.

Rehabilitation begins around treatment and can include exercises, mobility aids and plans for school, work or home activities. Tell the team about every medicine and supplement. Protect the affected limb as advised, since a tumour can weaken bone, and do not massage or manipulate a suspicious lump.

Contact the treating team promptly if pain or swelling rapidly worsens, the limb becomes difficult to use, or a new lump enlarges. Sudden severe pain, a snap, deformity or inability to bear weight needs urgent assessment for a possible fracture.

While receiving chemotherapy, use the unit's emergency number for fever, shaking chills, breathing difficulty, unusual bleeding, confusion, severe dehydration or symptoms of an allergic reaction. Do not wait for a routine appointment, because infection and treatment reactions can progress quickly.

Ask who should perform the biopsy, what operation is proposed, how reconstruction could affect movement, and what rehabilitation will involve. It is also useful to discuss fertility preservation before chemotherapy, expected effects on daily life, and the symptoms that require the treatment unit's emergency line.
