How bone infections are classified

These categories overlap. A clinician may describe the same case as long-standing, introduced through a wound, and associated with fixation hardware. The labels help explain which tests, antibiotics, wound procedures, or operations may be appropriate.

Main patterns of osteomyelitis

Acute osteomyelitis develops over a shorter period and may cause prominent pain, swelling, warmth, or fever. Chronic osteomyelitis persists or returns and can involve dead bone, a draining sinus, or symptoms that fluctuate. Timing alone does not show how severe an infection is.

How the type is identified

Tell the clinician about recent injuries, wounds, operations, implants, infections elsewhere, and previous antibiotic use. Examination and imaging show the site and extent, while blood or tissue samples can identify the organism. A sample taken directly from the affected area may be more informative than a surface swab.

Treatment is matched to the pattern rather than the label alone. Targeted antibiotics are central; drainage, debridement, implant management, wound coverage, or reconstruction may be added when the structure of the bone and surrounding tissue requires it.

When faster assessment is needed

Seek prompt assessment for fever with worsening focal bone pain, spreading redness, increasing swelling, pus, exposed bone, or a wound that is becoming deeper. A painful or draining area over an implant also needs timely review.

Emergency assessment is appropriate for new confusion, faintness, breathing difficulty, rapidly changing skin colour, or severe illness. Reduced sensation can hide pain, so a new foot ulcer or unexplained swelling should not be judged by discomfort alone.

Ask whether the infection appears acute or chronic, how it likely reached the bone, and whether an implant or nearby wound is involved. You can also ask which sample will guide treatment, whether damaged bone is present, and what changes would signal that the plan needs adjustment.
