Bone infection in the leg: what to know

Short answer
A bone infection in the leg is osteomyelitis, a condition that requires timely diagnosis and treatment to limit bone and tissue damage.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Condition
- Osteomyelitis is infection within bone.
- Diagnosis
- Clinical findings, imaging, blood tests and samples may all contribute.
- Care priorities
- Treat the organism, remove unhealthy tissue when needed and preserve leg function.
The short answer
Bacteria can enter after an open fracture, surgery, a deep wound, or spread from another part of the body through the blood. Care often combines targeted antibiotics with management of any wound or unhealthy tissue.
What happens in the leg
Osteomyelitis can involve the shin bone, thigh bone, or another bone near an infected wound. People with the condition may develop deep localized pain, tenderness, warmth, swelling, redness, drainage, fever, or reduced use of the leg. A long-standing infection can form a channel that intermittently drains through the skin.
The diagnostic work-up may combine examination, blood tests and imaging. A tissue or bone sample can help identify the organism and guide antibiotic choice, particularly when surgery is required.
How to respond
Arrange prompt medical assessment for ongoing bone pain with warmth, swelling, a wound, or drainage. Cover an open area with a clean dressing and follow any weight-bearing restrictions already provided. Avoid squeezing a draining tract or taking antibiotics left from an earlier illness, as this may delay appropriate sampling and treatment.
Management depends on whether the infection is recent or long-standing, the amount of affected tissue, limb stability, circulation, and overall health. Antibiotics may be given by vein or mouth. Surgery may drain a collection, remove infected tissue, improve stability, or reconstruct a bone defect once the team considers that appropriate.
When the situation is urgent
Go for urgent assessment if redness or swelling is spreading quickly, pain becomes severe, drainage has a strong odour, skin turns dark, or fever occurs with a leg wound. Confusion, faintness, rapid breathing, or feeling acutely unwell can signal a serious systemic response to infection and needs emergency care.
After leg surgery or an open fracture, new drainage, wound separation, increasing warmth, or a sudden loss of function should be reported promptly to the treating service.
What to clarify with the care team
Ask which bone and surrounding tissues are involved, how samples will guide treatment, whether the leg is stable for weight-bearing, and how wound progress will be monitored. If surgery is proposed, clarify its aim, the expected rehabilitation plan, and which symptoms should bring you back sooner.

Questions people ask
Can leg osteomyelitis come back?
It can recur if infection remains in poorly perfused or dead bone. New pain, swelling, redness, or drainage after treatment should be assessed rather than watched at home.
Does every bone infection in the leg need surgery?
No. The need for surgery depends on factors such as an abscess, dead or unstable bone, an infected implant, wound condition, and response to antibiotics.
Can physiotherapy begin during treatment?
Rehabilitation may help preserve safe movement and strength, but loading must match the bone's stability, wound status, and surgical instructions. The treating team should set those limits.
