Ultrasound and bone densitometry

Quantitative ultrasound is usually performed at a peripheral bone such as the heel. It measures how sound waves travel through the tested area, providing information related to bone strength rather than a DEXA T-score from the hip or spine.

The test is portable and does not use X-rays, which can make it useful for an initial risk check in some settings. Its result depends on the device, the bone tested, positioning, and local soft tissue, so measurements from different machines are not automatically interchangeable.

Central DEXA directly measures bone mineral density at clinically important sites, particularly the hip and spine. A clinician may recommend DEXA when an ultrasound result or your health history suggests that a fuller fracture-risk assessment could change care.

Keep the report and note which device and body site were used. Review it with a clinician alongside previous fractures, falls, regular medicines, menopause history, nutrition, smoking, alcohol intake, activity, and medical conditions that can weaken bone.

Avoid starting bone medication from a screening ultrasound alone. The next step may be lifestyle support, formal DEXA, investigation for contributing illness, or no immediate testing, depending on the overall assessment.

An ultrasound screening result is not an emergency. Prompt evaluation is appropriate when pain persists after a minor fall, you lose height unexpectedly, or new focal back pain develops. Obtain urgent help if an injury leaves you unable to bear weight, there is visible deformity, or severe back pain is accompanied by weakness, numbness, or bladder or bowel disturbance.

Ask what the ultrasound measured, whether the device has a clinically useful reference range, and whether central DEXA would add information. Also ask how your fracture history and medicines influence the decision and whether follow-up should use the same method.
